LICENSED PRACTICAL NURSE: CITIZENS CARE AND REHABILITATION CENTER

 

These nursing positions provide comprehensive resident care at Citizens Care and Rehabilitation Center (CCRC). Direction may be given to Nursing Assistants and other staff members. Supervision is received from the Registered Nurse Supervisor, Assistant Director of Nursing, Director of Nursing and/or the CCRC Executive Director.

Examples of Work

    • Administer medications and treatments to residents per doctors’ orders and/or CRNP orders, up to and including treatment for Stage IV wounds (necrotic or bone exposed wounds)
    • Perform neurological assessments, fall prevention assessments, wound prevention assessments, and all other required assessments to maintain residents at their highest functional levels
    • Provide appropriate documentation for residents receiving skilled services, as directed
    • Assess all residents for pain/discomfort a minimum of once each shift; prepare

    • Plan, administer, supervise and update resident care plans for each resident within the scope of their abilities
    • Observe, evaluate, record and report the symptoms, reactions and progress of each resident
    • Keep family members and physicians informed of residents’ changing condition and provide support when needed to any family members
    • Make rounds of the nursing units throughout the shift; ensure that medical orders and facility policies and procedures are followed
    • Administer and supervise special medications and treatments per doctors’ orders and/or CRNPs’ orders
    • Monitor work performance of Nursing Assistants and Certified Medicine Aides
    • Initiate intravenous therapy and provide intravenous medication as order by the physician
    • Perform other related duties as required

Qualifications and Requirements

    • High school graduation or the equivalent
    • Current LPN license issued by Maryland State Board of Nurse Examiners (current license must be maintained throughout term of employment
    • Current CPR certification
    • Knowledge of current nursing methods and practices
    • Ability to effectively direct the work of others
    • Ability to effectively evaluate residents’ condition and appropriately respond to immediate needs and emergencies
    • Strong and effective spoken and written (English) communication skills including the ability to clearly convey information to residents and co-workers, and to accurately record medical information, maintain related records and perform other clerical tasks
    • Ability to develop and maintain effective working relationships with co-workers and other medical professionals, residents and their family members and the general public
    • Physical requirements include the ability to move/carry equipment and to lift, move and assist residents as needed
    • This position and the physical workplace may include exposure to air contaminants, hazardous chemicals, infectious waste, diseases and medical conditions, (e.g. Tuberculosis, AIDS virus, Hepatitis B virus, etc.)

Preference May Be Given For

    • Experience using an Electronic Medication Administration Records system
    • Minimum 2 years work experience in a nursing home or related facility
    • Experience caring for ventilator dependent residents
